Graylog 5.0 Compatibility

Adds compatibility with Graylog 5 +

Issues fixed in this release:

• Plugin not appearing in EventNotification list
• Plugin show "unknown type" when viewing alert details

Redesigned for 3.0+ callback support

This release has been tested on my dev and production instances however there may be a few bugs. Plugin can be accessed under the "Pushover notification" type in the notifications menu.
