- Starting dose : 0.15-0.20 mg/kg/day - 2 divided doses
- Ctrough 8-16 ng/mL (Whole blood)
- Cyclosporin
- Starting dose : 8-10 mg/kg/day
- Ctrough 200-400 ng/mL (Whole blood) - first 3 months
- Ctrough 100-300 ng/mL (Whole blood) - thereafter
