I've noticed this also happening on "Add upload" page while searching for artist (I suppose it's not very different from "add format" page).
Main upload page is fine.

Also, I guess, I should note that when I try to enter a direct link to discogs page, for example, instead of search query, while on artist add upload/add format pages, it also hangs on "Please wait..." with only names entered.
So, this all means that something that isn't on main upload page but is present on other pages somehow breaks the script?..
