During the installation of Course Manager Configuration plugin on Moodle 3.9, with debugging set to DEVELOPER mode, there was an error. This pull request addresses the issue

Here is the error message that was appearing:

- Block is no longer visible if user is not authenticated.
- Fix-2: Plugin setting page, specifically to enable/disable request control option.

Updated
- Refactored block_cmanager.php
- Block can be placed on any page.
- You can no longer add more than one instance of the block to a page.
