The Survey App is HTML5 based application, you can use to survey using great Survey JS (https://surveyjs.io). Based on only 2 html5 pages, with single javascript this is complete in itself. Anybody can use this app and make customized Survey using survey editor to take customized survey. Survey result uses chartjs api to build beautiful graphical representation Data will remain persist, next time you open up the app. The app uses ArcTop Labs : MasterDataNode Data Solution feature in order to store the Survey data.

Create an App after login, copy and paste App Name in arctoplabs.js, index.js file appName variable
Copy and Replace the access_token (available in your login) in arctoplabs.js, index.js file access_token variable
